Forms & Records

Forms your instructors will actually use.

Design an assessment once, publish it, and let instructors grade it on the iPad. Every record keeps its form, its grades, its signatures and its history, for as long as the authority asks.

What is inside

From the item catalogue to the audit dossier

Everything a training department needs to design, grade, review and keep an assessment, plus the reports crew members file about their day.

Item catalogue

One list of tasks and manoeuvres, shared by every form. Rename a task and its history stays whole.

Form builder

Sections, items, rating scales and pass rules, with a preview that looks exactly like the iPad.

Competency or task grading

CBTA competencies, classic task grades, or both on the same form.

Published versions

A published form is locked. A new version never changes the records made on the old one.

Review queue

Every submitted form waits for a reviewer and an approver before the result counts.

Audit dossier

The whole check on one printable page, ready for the inspector.

Session feedback

Trainee and instructor rate each session, anonymously, without touching the result.

Crew e-forms

Safety and fatigue reports with the same review flow and their own access rules.

Designing the form

Built the way your department grades

Form sheet with sections

Your framework, your scale, your rules

Name the form, choose whether it grades competencies, tasks or both, attach your competency framework and rating scale, and decide what happens when a grade falls below the line. No developer needed.

  • Your own competency framework, or the IATA and ICAO ones
  • Rating scales with a clear pass level
  • A preview that looks exactly like the iPad

Sections, items and the fields aviation needs

Aircraft, route, simulator, altitude, fuel and times are built in. Show a section only when it applies, for example when a go-around was flown, and it stays out of the result when it is not.

  • Items drawn from one shared catalogue
  • Sections that appear only when they apply
  • Briefing sheets and checklists attached to the form

Fig. 04The pass rule

A pass rule in plain English

The form tells you, in words, when it fails: any item below the line, a section that must pass, a decline over consecutive checks. Comments become mandatory where your policy says so.

  • A final assessment with its own scale
  • Mandatory comments below a threshold
  • Notifications tied to the form

Versions you can defend

Publishing locks the form. A later change creates a new version, and older records always show the version they were graded on.

  • Who published what, and when
  • Duplicate, adjust, publish, archive
  • Nothing edited in place
Keeping the record

Two signatures, then it is history

Rubber stamp on an approved record
Fig. 06Review and approval

Two signatures before a result counts

A reviewer checks, an approver approves, the trainee acknowledges. Grades cannot be changed afterwards. If something is wrong, the form goes back with a note and only the inputs reopen.

  • Return for correction, with the fields to fix named
  • A reason recorded for every rejection
  • The complete history behind each record
Dossier folder
Fig. 07The audit dossier

The record is the archive

The audit dossier puts the whole check on one printable page: who graded whom, on which form version, every rating and comment, the threats and errors recorded, the signatures and the history.

  • Print it or save it as PDF
  • Nothing to reconstruct before an inspection
  • The same layout every time
